AMITA Sage Medical Group

Health Services

AMITA Sage Medical Group (AMITA) is a community health care provider with an office conveniently located directly adjacent to DePaul’s Lincoln Park campus on Fullerton Ave.

Students who live in the residence halls are automatically enrolled in the program and charged a $60 quarterly fee. Students living in the residence halls can opt out of the program, and students who do not live in the residence halls can opt into the program. See below for details.

Note: Health Services is not health insurance. See below and the Health Insurance web page at the left for more information.

Students who pay the quarterly $60 health services fee are treated as current patients at AMITA, and office visits for minor illnesses and injuries and basic lab work are covered by the fee.

Students who do not pay the health services fee will be treated as new patients at AMITA. During busy times, AMITA might not be able to accommodate new patients, but will instead provide a referral to another provider in the area.

Acute Versus Chronic Treatment

The health services fee is designed to cover acute illnesses and injuries. For students with chronic illnesses, Presence can treat an acute episode; however, long-term monitoring and treatment is not covered by the fee.

STD Testing

The health services fee does cover STD testing when a student is experiencing symptoms. Students' sexual health is an important component of their development, and AMITA is an appropriate support and referral source for students. For more information about STD testing in your area, please visit It's Your (Sex) Life.

Prescriptions provided to students are NOT covered by the fee. Pharmacies adjacent to campus accept a variety of prescriptions drug cards as well as cash payments.

AMITA cannot refill prescriptions for the treatment of ADD/ADHD. Other ongoing prescriptions, such as Accutane, antidepressants and bipolar medications, cannot be refilled by physicians at AMITA.

Parents and Guardians

AMITA welcomes parents and guardians to call with general concerns or questions. While specific questions regarding diagnosis or treatment may not be shared without the consent of the patient, the staff will make every effort to assist parents and guardians because they understand how difficult it can be to be far away from a sick student.

How do I Pay the Health Services Fee/Dates of Coverage

Students who live in a residence hall (except the University Center) automatically pay the $60 (per quarter) health services fee as part of their on campus housing. Students who commute or live off campus can pay the health services fee with a credit card by logging into Campus Connect. Go to: Main Menu - Self Service - Student Resources - Sage Medical Registration. If you need to pay the fee by cash or check you must visit DePaul Central.

Dates of Coverage:
Fall quarter: September 1, 2018 - January 1, 2019
Winter quarter: January 2, 2019 - March 23, 2019
Spring quarter: March 24, 2019 - June 8, 2019
Summer (optional): June 9, 2019 - August 31, 2019

How do I discontinue coverage?

Before choosing to opt out of the health services fee, consider the student's access to local health care and insurance coverage. AMITA can provide easy and affordable access to basic health care.

To discontinue the health services fee, residence hall students must present a copy of their DePaul ID card and proof of medical insurance (copy of an insurance card or a letter from the insurance company by two weeks into a quarter, with no refunds after that deadline). On the copy, PRINT: student name, residence hall with room number and DePaul ID number. Present documents in person or send to the Dean of Students Office, 2250 N. Sheffield Ave., Suite 307, Chicago, IL 60614, by the following deadlines:

  • Fall quarter: Friday, September 27, 2019, to be dropped for fall, winter and spring quarters
  • Winter quarter Friday, January 24, 2020, to be dropped for winter and spring quarters
  • Spring quarter: Friday, April 17, 2020, to be dropped for spring quarter

Once a student has submitted a copy of an insurance card at the beginning of the year, there is no need to present it every quarter. This is a yearly process. The bill will initially show a charge for "medcare" each quarter, but the account will be credited following the first two weeks of each quarter.
