Research Support Facility (RSF) Forms

Form Description
Research Support Facility Access Request Form


To be used to request access to the RSF or to remove an individual's access privilege. Requests for adding or removing access are to be completed by the direct supervisor of the individual, i.e, Chair, faculty researcher (for student requests).
The form should be submitted via email to jkirin@depaul.edu  or to the Director of the RSF mailbox located in the Department of Biological Sciences.
