Opening PDF Files

If you click on one of the fillable PDF forms (i.e. RSF Access Request Form, IACUC Alternative Training documentation, Request for Approval of Alternate IACUC Training Methods) and you get a message that states:

Please wait… If this message is not eventually replaced by the proper contents of the document, your PDF viewer may not be able to display this type of document…

Check whether you have a pop-up window asking you to open the document with a different viewer. If you do, then select Adobe Reader or Adobe Acrobat (if you have that software). If you do not have Adobe Reader, you may download a free version at:get.adobe.com/reader/otherversions.

If you have the latest version of Adobe Reader, then this message means your web browser (i.e., Chrome, Firefox) settings need to be reset so PDFs can open in Adobe Reader and not in a default (Preview) setting. It is best to not use Chrome for our PDF forms. Firefox works best as a browser for these forms.

Protocol Submission Forms

Form	Description
FORM100-Version 2-2019-For Posting.doc	To be used for all new protocols and to be used when investigators need to resubmit (3 year anniversary) for de novo review
Amendment: Revision Form 200	To be used for requesting changes to an approved protocol
Renewal Form	To be used annually for approved protocols in order to report back yearly. Please Note: You must receive an annual renewal in order to continue your project
Change in Personnel	To be used to add and/or remove personnel from an approved protocol

Training Submissions Forms

Research Support Facility (RSF) Forms

Form	Description	Version
Research Support Facility Access Request Form	To be used to request access to the RSF or to remove an individual's access privilege. Requests for adding or removing access are to be completed by the direct supervisor of the individual, i.e, Chair, faculty researcher (for student requests). The form should be submitted via email to jkirin@depaul.edu or to the Director of the RSF mailbox located in the Department of Biological Sciences.	01/29/2024
