This form is intended for those who are responsible for the design, conduct, or reporting of a sponsored project for which DePaul University is requesting or receiving funding.
Completion of the form is required:
- When a proposal is submitted for funding.
- Annually for the duration of a multi-year sponsored project.
- When a change in circumstances causes any of your answers to change from “no” to “yes.”
