If you would like to lower your Dining Plan, you must do so no later than the first two weeks of the academic quarter.

This Form is for non-residents only.
This form can be used to add a meal plan or change current and future meal plans.
To cancel your meal plan, please visit the Student Centers Office in person in suite 303 of the Lincoln Park Student Center.
To add dollar value to your card, please go to Campus Connect, select the Meal Plan Online link, and follow the steps.
If you would prefer to complete a hardcopy form, please use the PDF DIning Plan Form. Please either fax the completed hardcopy form to (773) 325-7347 or bring the completed form in to the Lincoln Park Student Center Room 303.
